- Children must be 5 years of age on or before September 1, 2010.
- Your child’s certified birth certificate must be presented as proof of age.
- Your child’s social security card is needed.
- Parents are required to present three forms of residency. (click here for residency information) If you currently have children in the district, proof of residency will not be needed.
- School fees ($50.00 per child) will need to be paid. If your child will be eligible for free or reduced lunches, you may complete a fee waiver.
- Children must have a completed physical examination form with an up-to-date immunization record before the first day of school.
- Illinois Transfer Form if coming from an Illinois school

Returning Students: A preregistration packet was sent home with each student with the third quarter report card. The registration documents in the envelope must be returned to the district office as soon as possible.
