Nurse Notes

The state of Illinois has changed many requirements for students attending school in the state.  These changes are state law and must be followed by all Illinois school districts!

PHYSICAL EXAMINATION AND REQUIRED IMMUNIZATIONS

To be completed within one year prior to entering preschool, kindergarten, 6th grade and 9th grade, or ages 5, 10, and 15 for non-graded classes.  5th Grade students and preschool students are required to have 3 doses of the Hepatitis B vaccine.  All students transferring into Illinois from out-of-state or out-of-country are also required to show proof of a physical examination and required immunizations.

DENTAL EXAMINATION

All students entering kindergarten, 2nd grade and 6th grade must present proof of having been examined by a licensed dentist.  Parents have been sent numerous letters and reminder memos regarding this requirement.  Report cards will be held until the exam form or the waiver from is turned in to the nurse.  Please contact the school nurse with any questions.  The state deadline is May 15th.

MEDICATION PICK-UP

All medications that were used during the current school year will need to be picked up by June 3rd.  Please note that any medications that are not picked up by an adult will be disposed of per district policy.  Please contact the nurse with any questions.
