Who needs a physical?
o   All students entering an Illinois school for the first time (including pre-K & all out of state transfers)
o   Kindergarten
o   6th grade
o   9th grade
o   Athletes must have a completed sports physical on file every year before they can attend practice.

School physicals are due on October 15.  Physicals must have a complete set of required immunizations for the student to be in compliance.  Students who have not met the physical exam requirement AND ISBE/ACIP required vaccinations will be excluded from school until those requirements are met.

 Who needs a dental exam?
o   Kindergarten
o   2nd grade
o   6th grade
o 9th grade
Dental exams are due on May 15 of the school year.

 Who needs an eye exam?
o   Kindergarten
o   All students entering an Illinois school for the first time (excluding pre-K).
Eye exams are due on October 15.
